  • Meets Local Coverage Determination (LCD)
    Hospice eligibility based on meeting Local Coverage Determination (LCD) involves adherence to the specific criteria established by the Medicare program or other relevant insurance providers. These criteria typically include a prognosis of a life-limiting illness with a life expectancy of six months or less, as certified by a physician. Additionally, the patient should elect to forgo curative treatments and opt for palliative care focused on symptom management and enhancing quality of life.
  • Meets Most LCD Guidelines and Documented Decline
    Individuals who meet most of the Local Coverage Determination (LCD) guidelines but also exhibit a documented decline in health are eligible for hospice care. This recognizes that some patients may experience a significant deterioration in their health status that might not fully align with all specified criteria. The documented decline serves as additional evidence, demonstrating the progression of the life-limiting illness and the increasing need for specialized hospice care services.
  • Meets Most LCD Guidelines and Additional Co-Morbidities
    In this context, co-morbidities refer to the simultaneous presence of multiple health issues alongside the primary life-limiting illness. Recognizing the impact of these additional health challenges on the overall well-being of the patient, hospice care is extended to address the diverse needs arising from both the primary illness and the co-morbidities.
  • Physician's Clinical Judgment
    In situations where a patient's eligibility may not precisely align with the outlined criteria in the Local Coverage Determination (LCD), hospice eligibility can be determined by a physician's clinical judgment.
